New Referrals:

Referrals to the Behaviour Support Service (BSS) can be made only by the school or setting by using the Behaviour Support Referral Form found in the 'Related Documents' section of this page.

Once completed, the form must be renamed using the following naming convention:
830XXXX_BSSfaosuzannbanksYYYYMMDD (XXXX should be replaced with the school's 4 digit DfE number)
The form must be uploaded via the School Secure Area on Perspective Lite.

Please check that you have completed all of the form fields. Any referrals submitted with missing information will cause a delay in processing and may result in the application being refused. NB: Parents must give their permission for Behaviour Support involvement.

Resuming a referral:

Pupils can be re-referred within the same setting within a period of 12-months after closing using the Behaviour Support Resume Referral Form found in the 'Related Document' section of this page.

What happens next?

We aim to process your referral within 3 to 4 weeks from receipt. Once processed, you will receive notification and be asked to complete a short online questionnaire. Your allocated Behaviour Support Teacher will contact you within 7 days following completion of the questionnaire.

Purchased Hours

The Behaviour Support Service (BSS) is a DCC traded service. In some cases, where pupils meet certain criteria, there is no additional cost for support. Please contact our admin team or your Behaviour Support Teacher for more information.
