The annual review is the statutory process of looking at the needs and provision specified in an Education, Health and Care (EHC) Plan, and deciding whether these need to change.

The first review of the EHC plan must be held within 12 months of the EHC plan being finalised. Subsequent reviews must be held within 12 months of the previous review.

Please see paperwork needed to complete the Annual Review in related documents of this page. Please submit completed documents using Perspective Lite.

See related documents section for an overview of the level of contribution that NHS services in Derbyshire can provide in support of annual reviews.

If you have any questions or need support and advice, please contact Derbyshire Information Advice and Support Service (DIASS) on 01629 533668 from 9.30am until 3pm.
