An EHC plan is a legal document that describes a child or young person's special educational, health and social care needs. It explains the additional provision that will be given to meet those needs and how that provision will support the child or young person to make progress so they can achieve what they want to in their life.

The assessment and planning process will put children, young people and families at the centre of decision making. The process is 'person-centred' and focuses on what is important for the child or young person what they want to achieve now and in the future. To do this it brings together a range of people who know and understand their needs and aims to provide greater choice and control.

Is the Education, Health and Care Plan reviewed?

The EHC Plan will be reviewed every 12 months. The review will focus on the child or young person's progress towards achieving the outcomes detailed in the EHC plan.

The review will involve parents, the child or young person, the setting or school, involved agencies and will look at whether the outcomes and resources in the plan are still relevant and consider changes if needed.

Which schools can be requested in the EHC Plan?

When a draft EHC plan has been prepared for a child or young person, the local authority will inform the parent or young person that they can request a particular school or other institution to be named in the plan.
